Nurse Registration Requirements:
- Filled up Oath Form or Panunumpa ng Propesyonal;
- Community Tax Certificate 2012 or Cedula;
- Two (2) pieces passport-size pictures (colored with white background and nametag);
- One (1) piece 1”x1” picture (colored with white background and nametag);
- One (1) piece documentary stamp;
- Original and photocopy of NSO Birth Certificate (original copy for presentation purposes only); and
- Initial Registration Fee of P600.00 and Annual Registration Fee of P450.00 for 2012-2015.
July 2012 NLE results were issued last August 23, with 27,823 new nurses making it to the list. PRC Baguio Regional Office coordinated and supervised the NLE in the cities of Baguio, Dagupan, Cabanatuan, and Laoag. In Baguio, 2,215 of 4,917 (45%) examinees successfully passed the July 2012 NLE. In Dagupan, 891 of 2,385 (37%) passed while in Cabanatuan, 657 of 1,976 (33%) examinees passed. In Laoag, 712 of 1,409 (51%) examinees passed.
